Below are samples of an HTML and TEXT email. The HTML version looks and is formatted much like a web page. The TEXT email provides all the same pertinent information in a simple text format any email client can read.

If you choose the HTML version, please ensure your email client supports HTML. Recent versions of email clients do support HTML (e.g. Microsoft Outlook Express, Outlook 98 and beyond), while older versions may not (e.g. Outlook 97). Web-based email services such as Yahoo and Hotmail also do support HTML.

If you do choose HTML and your email client does not display it accurately, simply return to the Action Request Form page - unsubscribe from the HTML version and then repeat the process to subscribe to the TEXT version.
